"Custody 2 Community" was created by Correctional Health Treatment Centers, Inc., a non profit community-based organization, with a mission to increase the access to care for the justice involved community. Our vision is by addressing individual health needs, healthier communities are created, which reduces recidivism, and enhances public safety.
"Custody 2 Community" is a strategically located Re-Entry Hub designed for those exiting incarceration as a single location to access health, housing, and care related services + connections. Staffed with peer lived experience Community Health Workers, Lead Care Managers, and Housing Navigators, we offer a welcoming environment for our justice-involved populations as we garner trust and connection.
